ORGANIZATIONAL PROFILE

Partners in Health (PIH) is a non-profit organization based in Boston,
Massachusetts, USA, whose mission is to provide a preferential option for the
poor in healthcare. Through service delivery, training, advocacy and research,
PIH works globally to bring the benefits of modem science to those most in
need and to serve as an antidote to despair. PIH has programs in Haiti, Peru,
Mexico, Russia, Rwanda, Lesotho, Malawi, Sierra Leone, Liberia, Kazakhstan and
Boston.

PIH began working in Malawi in 2007, where today we support the Ministry of
Health (MOH) to save a catchment area of 165,000 through 12 health centers and
two hospitals in partnership with our sister organizations.

Abwenzi Pa Za Umoyo (APZU) is in Neno District and we seek to recruit suitably
qualified and experienced candidates for the following position

Position Chronic Care Manager

Reports to NCD Physician Specialist

Commitment Full Time (Monday-Friday)

Working Hours Regular Office Hours (40Hrs/Week)

Location Neno, Malawi

Position Description Overview

The chronic care manager takes responsibility for the clinical care,
mentorship, team management and leadership of chronic care clinics in Neno
District Hospital and the health centers in Neno District. Chronic care
clinics takes care of patients with chronic illness enrolled in longitudinal
care including but not limited to patients with HIV and with non-communicable
diseases (NCDs) such as hypertension, diabetes, asthma, epilepsy, heart
failure, etc, and this person will help this program become a model of care
for this population. This includes efforts to improve, measure, and
troubleshoot the program. The chronic care manager collaborates closely with
the HIV Manager and NCD physician as well as any other programmatic staff
supporting this patient population across the spectrum of care (e.g.
Nutrition, Mental Health, Pharmacy, and inpatient teams). He/she also
collaborates closely with DHMT, academic institutions, MOH and other partners
within Neno and outside.

Responsibilities and Duties

Clinical care, programmatic implementation and medical education
• Provide supervision, oversight, mentorship and consultation for clinical
teams serving chronic care patients with NCDs, mental health and HIV in
integrated clinics and inpatient wards in Neno District
• Work with the DHMT and MOH coordinators to improve care in chronic care
programs in Neno District.
• Support the delivery of sessions of clinical case presentations, medical
literature review, mortality & morbidity review meetings, clinical topic
sessions and other medical education programs to integrated chronic care staff
and potential learners in Neno.
• Collaborate with M&E department to track integrated chronic care program
indicators with making data available and actionable to all clinical personnel
through regular review meetings and clinic operations
• Support the implementation and iterative improvement of electronic medical
records for chronic care patients including design, testing, and oversight of
clinical use and data entry for high quality data capture
• Provide support to formulary development, planning, quantification and
procurement of medications and other medical commodities (lab reagents,
glucometers, Aic meters, etc.) while maintaining close coordination with
existing national supply chain
• Support for development of clinical capacity in diagnostic services
necessary for all chronic care.
• Work with the NCD team, MOH and national stakeholders to develop the
advanced NCD clinic in Neno into a national PEN Plus center of excellence.
This will require educator skills, formal lectures, clinical mentorship,
review and development of didactic materials, mentorship guides and assessment
tools.
• Work with the HIV team, Department of HIV and AIDS, local Moll teams and
national stakeholders to iterate on implementation of the Advanced HIV Disease
program with care delivery, didactic and clinical education, mentorship,
oversight and continuous improvement
• Lead primary care facility in-charge and other staff mentorship program for
NCD and HIV care
• Work with the NCD and HIV teams to design and implement an annual integrated
chronic care workplan with monitoring and adjustment throughout the year.
• Attend cross site chronic care meetings including mental health, NCD and HIV
teams.
• Work in a dynamic team environment, with a respectful and supportive
attitude, along side both MOH and Pill clinical, community and administrative
staff
• Other clinical duties as assigned including potential call responsibilities

Research and quality improvement
• Support the design, implementation, and dissemination of quality improvement
projects and research for Chronic Care patients and collaborate with other
teams on joint projects
• Support any research projects in chronic care patients alongside other
programmatic leadership
• Support the evaluation, development and dissemination of program impacts
through systematic program evaluation, presentations, conferences and other
forums

Management, Supervision and supporting partnerships
• Work with APZU leadership and district colleagues to develop and implement
long term plans for chronic care patients
• Manage chronic care budgets including trainings and projects
• Ensure effective communication and collaboration on projects between NCD
team, mental health team, HIV team, and the relevant MOH coordinators.
• Supervise and oversight of staff who work with chronic care patients with a
primary aim of direct and goal-oriented mentorship
• Build and maintain relationships with government partners, including the
Ministry of Health coordinators, district health management team (DHMT), and
other local and international organizations Assist in grant proposal
development, writing, submission and carrying out activities and
implementation with reporting to donors
• Lead other clinical initiatives under Clinical Program as required

Qualifications and requirements
• MD, MBBS or equivalent
• Board-certification in Internal Medicine and/or Master In Public Health will
be an advantage
• Minimum three years’ experience in working in resource-limited setting
• Experience in team oversight, managing clinical staff using data for action
and managing budgets will be an advantage
• Fluent in English and demonstrated proficiency in writing in English
• Fluent in Microsoft Office with ability to produce reports and documentation
independently
• Willingness to live and work full-time in the rural, hard to reach district
of Neno
• Ability to work well in a dynamic team environment alongside health workers,
support staff and patients from a variety of backgrounds.
• If Malawian, active registration with the Medical Council of Malawi by the
time of the agreed-to job start date.
